The Importance of Continuing Care
The transition out of residential treatment is one of the highest-risk periods in recovery. The structure, community, and clinical support that surrounded you on campus does not disappear overnight at Holland Pathways, it follows you into the next phase, gradually releasing as your confidence and skills grow.
Research is clear on this: the longer someone stays engaged in treatment and support, the better their long-term outcomes.

The Step-Down Continuum
Partial Hospitalization Program (PHP)
PHP is the first step after residential treatment for most clients. You will spend structured daytime hours, typically 8am to 5pm, continuing the clinical work you began during your residential stay, with our clinicians guiding individual and group sessions.
You return home or to sober living in the evenings, beginning the gradual integration back into daily life while maintaining intensive support.
Intensive Outpatient Program (IOP)
IOP offers continued clinical structure with more flexibility. You will attend three, three-hour sessions per week at Holland Pathways, continuing therapy, group work, and relapse prevention practice while having greater freedom to navigate work, family, and daily responsibilities.
IOP is where the tools you built in treatment get tested and refined in the real world.
Outpatient Program (OP)
Outpatient care is the final step in the clinical continuum, offering one group session per week and a monthly one-on-one appointment with your therapist. OP keeps you accountable, connected, and supported during the period of sustained recovery when the temptation to disengage from care is highest.

Alumni Program
Recovery is a lifelong path, and no one should walk it alone. Our Alumni Program exists to make sure that the community you built at Holland Pathways stays with you every step of the way in early recovery.
Led by our Alumni Coordinator, the program connects graduates with each other and with ongoing support, because the people who have walked this path are often the most powerful resource for those still walking it.
Alumni events, peer connection, and ongoing touchpoints keep the Holland Pathways community alive and active long after discharge.
Whether you completed our program a month ago or five years ago, you are still a part of this community.
Support for Families
The people who love someone in recovery need support too. Holland Pathways partners with The Family Recovery Foundation to provide families with free, accessible, compassionate support throughout the recovery process — including weekly support groups, one-on-one sessions with licensed professionals, and educational resources designed specifically for families navigating a loved one's recovery. Because healing the family is part of healing the person.

Discharge is not a finish line. It is a threshold. Holland Pathways will be with you on the other side of it, through every step of the continuum, through the alumni community, and through every resource we can put in your hands for the road ahead.
